New Patients: Register With Us

The practice has an open list and is registering new patients whether new to the area or not. 

You can register online or by asking for a registration form at reception. 

Following your registration you will be required to have a new patient check to enable your registration to be fully complete. An appointment will be offered with our nurse during your registration.

Medical treatment is available from the date of registration. Please contact reception for further information.

How to Register at Meir Medical Practice

If you would like to register to become a new patient at Meir Medical Practice, you can find out how to register as a new patient below.

How to Register as a new patient

You will need to complete the online registration form to register at our surgery. This form will ask questions including details of your previous GP surgery and basic health and medical information.

Although it is not mandatory, you may be asked to provide a copy of your ID for a proof of an address. For example, valid ID could include a photograph of your passport or driving licence. This is to check that you live in the local catchment area.

We will require a completed form for every member of your household who wishes to register. The application may take a few days to complete. If you have not heard back from the surgery within 5 working days, please contact the surgery by phoning 0300 123 0903 or email [email protected]

Registering as a non-English speaker

Registering as a Temporary Patient

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

If you are away from home, you can register as a temporary patient with Meir Medical Practice for up to three months. This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.

Register as a temporary patient

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered. 

Registering Children

Children under 16 need to be registered by their parent or guardian. You can do this by completing the new patient registration form, and child immunisation form.

You will need to provide a form of ID to prove that you’re their parent or guardian.

What to do if you’re registering on behalf of someone else

If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

Online Pre-Registration with the Practice

If you wish to pre-register click on the link below to open the forms. When you have completed all of the details, click on the “Submit” button to mail your forms to us. When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct.

Change of Address

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.

Please also let us know if your telephone number or email address changes.

Disabled Facilities

Disabled patient facilities are also available at this practice.
